Mini-Stim IVF: aka Modified Natural Cycle IVF

It is always interesting to see a publication advocating what you have been doing for some time.  The group at the University of Montreal recently published an article in Fertility and Sterility (2011: vol 96,  pp 1066-8) advocating  the use of modified natural cycle IVF as a first therapy for women with decreased ovarian reserve.  Their modified natural cycle IVF was rather similar to our Mini-stim IVF and they view this approach in much the same way as I do.   Thus Mini-stim IVF, mini IVF and modified natural cycle IVF are all synonyms.

An important part of our approach has been for me to directly perform a transvaginal ultrasound on new patients at their first visit to assess ovarian reserve.  This can save a couple months to years of a fertility evaluation.  It is not uncommon to find women in their early 30’s with decreased ovarian reserve.   Patients with decreased ovarian reserve who undertake appropriate therapy early often have a good chance of getting pregnant.  If they wait too long (perhaps by following simpler infertility algorithms for too long a period of time), they may never achieve pregnancy.   A reproductive endocrinologist is more likely than a general gynecologist to move quickly in the right direction, but as suggested by the above article, the optimal patient approach is not quite mainstream yet.

Getting Ready For Your First Visit to an IVF Clinic

You and your spouse have decided to take a leap and visit an IVF clinic. In-vitro fertilization can increase your odds of conceiving up to 45 percent. Since many clinics have wait lists, this will give you plenty of time to prepare for your first appointment.

Information to Keep Track Of

While waiting for the big day, you should take the time to record a few things.    Try to remember who long you have had unprotected intercourse without conceiving.  If there were other partners on either side, include that time.  Also how long have you been working with doctors on getting pregnant and what type of doctors were they?   Record who long it is from the beginning of one period to the beginning of the next period.  If your periods vary, try to provide an accurate range of days.  How long do your periods last?  Record the number of days (or range).  How many (if any) days of spotting proceed the start of genuine period symptoms (don’t count these as part of your period).   You may already be monitoring the above information; just be sure to bring it with you to your appointment.

Your fertility doctor will be interested in your medical history. Take copies of any past fertility and semen tests that have been performed, as well as any records of conditions that may affect your chances of conceiving.

Common Causes of Male Infertility

Did you know that male infertility is involved in about two-thirds of all infertility cases? It’s true. About one-third of infertility cases are caused by just the male. In another third of infertility cases, the infertility is caused by both the man and the woman.

With that in mind, it just makes sense to take a closer look at some of the common causes of male infertility. 

  • Abnormal sperm production—This can be due to a range of different problems, including genetic defects, infections, and undescended testicles.
  • Poor health—Male infertility can be caused by poor eating habits, drinking too much alcohol, taking drugs, being overweight, using tobacco, and other health and lifestyle issues.
  • Age—As men get older, they often become less fertile. Typically, men who are older than 40 are likelier to have infertility issues than younger men.
  • Sperm delivery issues—Things like premature ejaculation and blockage of the part of the testicle containing the sperm can prevent proper delivery of sperm.

Mini-stim IVF is utilized by several different groups of patients

There are three types of patients who utilize Mini-stim IVF in our practice and they have potentially different pregnancy rates with Mini-stim IVF as they do with conventional IVF. The best group for Mini-IVF is the group who could do IUI first, but move to Mini-stim IVF because it is a more efficient way to achieve pregnancy.  With problems like isolated male factor, the pregnancy rate with Mini-IVF is at least four times that of IUI (depending on how poor the sperm are).

The second group includes those patients who are candidates for conventional IVF, but choose Mini-stim IVF because of its benefits (cost, simplicity, less drugs, and fewer embryos created). Mini IVF is well suited for those patients concerned about creating viable embryos that may end up being discarded.  It is also well suited for those very concerned about multiple births as usually only one really good embryo is created. 

The third group includes women over 40 or those patients with decreased ovarian reserve. This latter group has a low success rate with IVF and Mini-stim IVF is often the preferred approach in our program.  Existing data suggests that the pregnancy rate is at least as high with Mini IVF as with conventional IVF and the cost difference is greatest because of the large amount of medication used to try to help this group with conventional IVF.  That large amount of medication may in itself cause problems in achieving pregnancy that can be avoided with a Mini IVF approach.
