Multi-cycle (3 for 2) discount package for conventional IVF
The best way to achieve pregnancy with IVF is to undertake an adequate number of tries.
The objective of this IVF package is to make IVF more affordable and to encourage the use of repeat cycles if needed to achieve pregnancy. Essentially this package requires that the patient pre-pay for (a few thousand less than) two IVF cycles. If needed to achieve pregnancy, a third cycle will be provided free. The charges covered are only those related to the IVF codes. For these codes, you may NOT use your insurance. However, you may use your insurance for other charges.
Included and excluded charges are listed in Appendix A. Medications, anesthesia costs, and costs associated with ovulation induction are not included. ICSI and assisted hatching are included when indicated to enhance the pregnancy rate. Cryopreservation is NOT included. If you do not have insurance coverage for the ovulation induction, we also have a package price for it. Although we cannot provide a package price for the medications you require, we can direct you to low cost pharmacies.
The charge for this package is $19,000. If you only require one cycle to achieve pregnancy, you will be refunded $9500. If you do not achieve a viable pregnancy (with a baby with heart beat continuing for at least 8 weeks after transfer) by the second cycle, a third cycle will be provided for you in which there will be no charges for the components listed in Appendix A. If a cycle is cancelled after aspiration, but before transfer, you will be charged for the actual services delivered and that cycle will not count as part of this package. If you wish to drop out of the program before achieving pregnancy (for example, after the first cycle), you will be refunded the program fee less all charges actually incurred.
If you are comparing our charges to those of another program, be aware that this package also covers all charges for ICSI, assisted hatching, and extended culture. In many programs these procedures are not part of the package price, but are commonly used in the IVF program and may significantly increase your costs. If you are comparing this program to that of our shared risk program, be aware that the shared risk program covers embryo freezing, storage, and cryo-defrost and transfer. For the multi-cycle package, these may be extra charges if you produce extra good quality embryos and wish to use them in a later cycle. For the shared risk program, there are no extra charges for these procedures and they do not count as a separate cycle.

Appendix A
The specific included components are:
- Oocyte aspiration and ultrasound guidance
- Oocyte identification
- Oocyte cleaning
- Sperm preparation
- Oocyte insemination
- Embryo culture
- Prolonged embryo culture if indicated
- Embryo transfer including ultrasound guidance
- ICSI if indicated
- Assisted hatching if indicated
The following components are NOT included:
- Evaluation and preparation for an IVF cycle
- Ovulation induction (ultrasound studies, blood tests/draws, and office visits)
- All medications including, for example, gonadotropins, lupron, progesterone, antibiotics, anesthetic agents, IV solutions/infusions
- All anesthesia charges
- Genetic studies including embryo biopsy (PGD) if desired
- Donor sperm or donor eggs
- Sex selection techniques
- Management of any complications
- Pregnancy evaluation (ultrasounds, etc.)
- Procedures or lab tests performed by other health care providers or at other places
- Surgeries other than oocyte aspiration
- Embryo freezing or cryo-storage
